| Cancellations |
| 24 hours notice is appreciated for weekdays and 48 hours notice
is appreciated for weekend appointments. Failure to show affects my ability to
offer my services to another client. |

| How much notice do you require if I need to reschedule or
cancel my appointment? |
| There is no penalty for cancellation, but I greatly appreciate
at least 24 hours notice. |

| How far in advance should I book my appointment to be sure I
get the time & day I want? |
| Please contact us at your earliest convenience to avoid
disappointment. |

| How far in advance should I arrive before my appointment? |
| Please arrive 15 minutes before any treatment. |

| What happens if I’m late for my appointment? |
| Please understand that if you are late for your appointment it
will be necessary to shorten or reschedule your treatment time so as not to
delay service for the next client. |

| Gift Certificates |
| Remember, your gift certificate will expire within three months.
However, if you make your appointment within those three months, the gift
certificate will still be honored. |

| Can I substitute a treatment of equal value for another
treatment? |
| Yes, however please understand that some treatments require more
time than others so I may need to reschedule your appointment. Please call at
your earliest convenience to change your treatment. |

| Am I expected to give a gratuity? |
| I am committed to giving you the best service possible.
Gratuities are greatly appreciate but are not required. |

| Where exactly are you located? |
| I am at 215 North 3rd Street about one block north of Main
Street behind the City Hall in Durant, Oklahoma. Rejuvenate is actually located
inside the Old Ice House Salon and Day Spa. The building is stucco with a terra
cotta tile roof. |

| Is there parking available? |
| Yes, there is parking in front and also behind the building. |
| |
| Do you have treatments available for men? |
| We have equal treatments available for both men & women. |
| |
| What should I bring with me when I visit Rejuvenate? |
| I encourage casual clothing. I do not provide robes or slippers,
but feel free to bring your own if it will make you more comfortable. If you
have received a gift certificate, please bring that with you. |

| Should I remove my make-up before coming for a facial or
microdermabrasion? |
| You do not have to remove your make-up before coming in for
either a facial or microdermabrasion.

| Can I put on make-up before leaving Rejuvenate after a facial
or microdermabrasion? |
| I do not encourage putting on make-up directly after a facial or
microdermabrasion because I would prefer your skin to breathe directly after a
treatment. However, it is possible to put on make-up. Just bring your make-up
with you or add a make-over to your appointment. |

| Will I have to fully disrobe for my massage? |
| I will always respect your privacy. Typically I ask clients to
disrobe to the level of their comfort (even if it is shorts and a t-shirt). You
will have complete privacy and plenty of time to disrobe and make yourself
comfortable on the massage table. During each treatment, your body will be
fully covered except for the area of the body involved. Please feel free to
